要旨　Advantage Fit^TMを用いたTVT手術，Monarc^TMを用いたTOT手術の手術手技を述べる。中部尿道のよい位置にテープを置くには，①Monarc^TMでは高い位置からのニードル刺入，②腟壁創の遠位寄りの位置決め，③最小限のポケット状剝離，の3点が大切である。膀胱誤穿刺が問題となるAdvantage Fit^TMの穿刺では，恥骨下で水平に泌尿生殖隔膜の抵抗を抜けた後，向きを大きく変え，恥骨後面の骨膜をすべらせる感覚で進める。腟壁誤穿刺が問題となるMonarc^TMの穿刺では，針先が待ち受けの示指より下にきたら，指を動かさず，ニードルの柄を立てて針先を持っていく。術後排尿困難の軽減には，カバー拡張操作，尿道引き下げ操作などを含め，テープ位置の調節に細心の注意を払う。
