副腎腫瘍の病理診断

要旨　副腎腫瘍の病理診断は，内分泌学的所見や病変の形態所見が非常に複雑であることから，他の臓器の病変よりも困難であることが多い。また，現時点で病理組織診断が可能なこととそうでないことを臨床/病理双方が明確に理解しておく必要がある。現時点で副腎皮質由来の腫瘍とそうではない腫瘍の鑑別は十分に可能であり，副腎皮質由来の腫瘍の場合には，その良悪性の鑑別も特別な場合を除いてWeissの指標を用いるなどして十分に可能である。しかし，現時点で必ずしも可能ではないのは，副腎皮質癌と病理診断がついたあとのその症例の生物学的悪性度の正確な同定と，褐色細胞腫の良悪性の鑑別である。
