副腎腫瘍のアブレーション治療

要旨　副腎腫瘍の標準治療は腹腔鏡を用いた外科切除であるが，近年，アブレーション治療の報告も増加している。アブレーション治療には，ラジオ波凝固療法や凍結療法，さらにはマイクロ波凝固療法といった熱アブレーション治療と，エタノールや酢酸を腫瘍に注入する化学アブレーション治療がある。良性の機能性副腎腺腫から悪性の原発または転移性副腎腫瘍が適応である。機能性副腎腺腫のアブレーション治療後，血清ホルモン値と症状の改善は95.8～100％の症例でみられると報告されている。一方，悪性副腎腫瘍に対するアブレーション治療により良好な局所治療効果が報告されているが，予後への貢献はいまだ明らかになっていない。
