要旨　ロボット支援前立腺全摘徐術（RARP）は，低侵襲手術として本邦でも急速に普及が進んでいる。広いワーキングスペース確保のため経腹的に行われることが一般的であるが，精管や精囊への到達法によって手術手法が異なる。後方到達法とは，手術早期に精管と精囊を前立腺の後方よりアプローチして処理する方法で，広い術野が確保されるため容易に精管と精囊を剝離できるのに加え，膀胱の後壁離断が安定して行われるので直腸損傷の危険性が低減し，手術時間の低減に寄与する。本稿では，RARPを経腹膜・後方到達法で行う実際の流れや注意点を中心に，われわれの行っている術式についての概要を述べる。
