要旨　ロボット支援前立腺全摘除術（RARP）は，アーム同士が干渉しないよう操作腔の広い経腹膜アプローチで行われることが多い。しかし，患者の既往歴によっては腹膜外アプローチが望ましい場合もある。特に，体格の小さな患者に対して腹膜外アプローチでRARPを行う場合は，アーム同士の干渉や切離した前立腺の処理方法などに課題がある。本稿では，これらの課題を克服するための工夫など，われわれが実際に行っている手術手技について報告する。
