要旨 根治的前立腺全摘除術では,癌の制御に加え,尿禁制に代表される生理機能の温存が重要である。ロボット支援前立腺全摘除術(RARP)は,優れた視認性と正確で繊細な操作が可能であることから,従来手術に比べて高い術後生理機能の維持が期待される。本稿では,術後尿禁制保持に重要な解剖学的構造の「温存」「再建」「補強」について,文献的に考察した。また,ロボット時代の前立腺癌手術における新たな取り組みや課題について概説した。
