要旨　尿路変向術は，筋層浸潤膀胱癌に対する根治的膀胱全摘除術に続いて必ず施行しなければならない術式であり，治療戦略を立てる際には尿路変向を含め検討を行う必要がある。体腔内にて尿路変向を含めたすべての操作を完遂する尿路変向術（ICUD）は，出血量や痛み，術後鎮痛薬の使用量の軽減，腸管露出回避による消化管機能の早期回復や体液不均衡の回避など，さまざまな利点が得られる可能性があるといわれている。本稿では，ICUDの手術適応，術前処置，手術時のセッティング，手術の実際，周術期管理などについて解説する。
