Ⅱ.膀胱癌に対するロボット支援膀胱全摘除術
文献概要
要旨 ロボット支援手術は,2000年にdaVinci® surgical system(Intuitive Surgical社)が米国FDAの認可を得て以来,世界中で普及しつつある革新的な手術方法である。米国では根治的前立腺全摘除術の85%以上がロボット支援下に行われ,すでに標準術式として確立している。膀胱全摘除術が術式的に前立腺全摘除術と類似していることから,ロボット支援前立腺全摘除術の導入でda Vinci®の操作に慣れた術者が,次のステップとしてロボット支援膀胱全摘除術(RARC)の導入を積極的に開始したことは当然の流れといえる。ここ数年でRARCの報告数は急激に増加し,安全性,短いlearning curve,導入期の良好な治療成績などが多数報告されつつある。本稿ではRARCの現時点での手術成績について文献的考察を加えて報告する。
