要旨　進行がん患者の意思決定支援の1つとして，shared decision making（SDM）という考え方がある。今後の治療・療養について，患者・家族と医療従事者があらかじめ話し合う自発的なプロセスをadvance care planning（ACP）と呼ぶが，ACPを実践するうえでSDMは重要な考え方の1つである。

　また，bad news telling（悪い知らせの伝え方）には一定のスキルがあり，SHAREプロトコールが日本では用いられている。
