要旨　後部尿道弁は，周産期から学童期まで，広汎な年齢の小児において，腎不全，腎形成不全，腎瘢痕，尿路感染，水腎水尿管症，VUR，排尿機能障害，尿失禁という，きわめて多彩な病態を呈する，小児泌尿器科学に最も特徴的な，かつ臨床的に重要な先天性疾患である。古くからよく知られている一方，内視鏡診断基準や経尿道的切開術について，総論的事項はともかくも各論的な事項についての標準化が不十分で，施設ごとにそれぞれ伝承された方法が行われているのが実情である。専門医の間にも，疾患概念の基本的認識の差が存在することが原因と思われ，これを正すために，自験例の臨床経験と文献情報を合わせて，最新情報をレビューした。
