腹腔鏡下膀胱拡大術—臍単孔式プラス5mmポート1本

要旨　5mmポート1本を追加した臍からの単孔式腹腔鏡下回腸利用膀胱拡大術を概説する。臍底部に2.5cmの切開を置き，単孔用アクセスポートを留置する。腹腔鏡下に，臍切開層から必要な回腸を体外へ引き出し，体外で遊離回腸を作成，遊離回腸の内腔洗浄後，脱管腔し，U字型に縫合する。遊離回腸を体内に戻し，再び腹腔鏡に戻り，腹腔鏡下に膀胱を大きく冠状切開し，遊離回腸と切開膀胱壁を縫合する。本法の創部は最小限度に小さくまた目立たず，そのうえ，腸管の処理が体外で行えるメリットは大きいと考えられる。
