性分化疾患に対する腹腔鏡下手術

要旨　性分化疾患診療において腹腔鏡検査を含めた腹腔鏡手術手技が役割を増している。現在の腹腔鏡の第一の役割は，性腺および内性器を確認する診断的腹腔鏡であり，卵精巣性性分化疾患でとりわけ有用である。腹腔鏡検査により詳細な形態的情報が得られると同時に生検診断が可能となる。第二の役割は性腺および内性器の摘出を行う治療的腹腔鏡である。性腺の摘出は決定された性に適合しない性腺を除去することで，不要な性ホルモンの産生を思春期以降抑制するとともに，Y染色体を持つ性腺異形成症例での性腺の悪性化予防を目的とする。
