要旨　非神経因性過活動膀胱（OAB）の原因には，①下部尿路閉塞，②加齢，③骨盤底の脆弱化，④特発性がある。下部尿路閉塞を有する男性の前立腺肥大症の約半数にOABが合併するように，女性では骨盤底脆弱を病因とし下部尿路閉塞を伴う骨盤臓器脱（POP）の約半数がOABを合併する。

　POPに合併したOABにも過半数の症例で抗コリン薬は有用であり，ペッサリーやPOPの手術によって約半数のOAB症状が消失する。

　POPに合併するOABには2通りあると考えられ，1つはPOPに別疾患としてのOABを合併している場合，もう1つはPOPそのものがOABを誘発している場合である。前者ではそれぞれの治療が必要になるが，後者ではPOPの治療をすることでOAB症状も消失することが多く，POP誘発OABはきわめてすみやかに改善する可逆的な原因によると推測される。
