骨盤臓器脱手術と尿失禁手術の同時施行：反対の立場から

要旨　骨盤臓器脱（POP）手術には，腹圧性尿失禁（SUI）の悪化リスクがあり，尿失禁手術の同時施行が論議されてきた。日本は，二期的施行74％，治療的同時手術（症候性SUI）20％，予防的同時手術（潜在性SUI）5％，超予防的同時手術（全例）1％と慎重な方針が多い。

　中部尿道スリング手術（TVT/TOT）が尿失禁手術の主流となり，症候性SUIのないPOPの腟式手術に，TVT手術併用の有無でRCT（OPUS試験）が行われた。12か月後の尿失禁は併用群27％，非併用群43％で，併用群0.6％，非併用群4.7％が尿失禁手術を要した。併用群は膀胱損傷，尿路感染症，排尿困難が多く，2.4％が排尿困難に対して手術を要した。

　同時手術には過剰医療，手術合併症増加の可能性がある。現時点では十分なインフォームド・コンセントのうえの二期的手術は妥当と考える。
