腹腔鏡下仙骨腟固定術（LSC）と下部尿路機能障害

要旨　腹腔鏡下仙骨腟固定術（LSC）はメッシュの利点を生かしつつメッシュ関連の合併症が少ない骨盤臓器脱の新たな治療法として注目されている。LSC前後の下部尿路症状に関して，術前の腹圧性尿失禁（SUI）の合併率は36％，術後の合併率は52％，術前に認めたSUIの25％は術後に消失，術前に認めなかった39％で術後新たなSUIを認めた。術前に過活動膀胱（OAB）を認めたのは全体の39％であったが，術後には21％まで減少していた。術後新たにOABを認めたのは5％であった。TVM手術後早期には尿勢の低下，残尿量の増加を認めたのに対し，LSC術後には認めなかった。LSC術後の下部尿路症状の変化に関してTVMとの違いを理解しておくことは，患者へのインフォームド・コンセントを行ううえで重要と考える。
