69．尿道損傷—ハシゴから落下して会陰部を強打し，痛みを訴えている患者です。

Point

●外傷後急性期は尿道カテーテル留置や損傷部の外科的修復を避け，膀胱瘻を造設して待機し，感染をコントロールする。

●外傷後3か月以上経過してから尿道を評価し，狭窄があれば尿道形成術を行う。

●外傷性尿道狭窄症に対する内尿道切開，尿道ブジー，尿道ステント留置は成功率が極めて低いだけでなく，狭窄を複雑化させるので，行ってはならない。
