前立腺癌に対する内分泌療法：歴史と概論

要旨　アンドロゲン依存性を持つ前立腺癌は両側精巣摘除，エストロゲン投与として内分泌療法が1940年代から開始された。その後，LH-RHアゴニスト，アンチアンドロゲン，LH-RHアンタゴニストの順で初期ホルモン療法のモダリティーが開発され現在に至る。去勢抵抗性の獲得メカニズムの理解に，古典的な内分泌“endocrine”から組織内内分泌“intracrine”の概念が重要となる。前立腺組織内でのステロイド合成，アンドロゲン受容体のシグナル伝達，さらに，リガンド非依存のアンドロゲン受容体活性化や，アンドロゲンそのものに依存しない増殖など，多面的な増殖メカニズムを理解して治療に臨むことが求められている。
