綜説

副腎偶発腫瘍—最新のマネジメント

要旨　副腎偶発腫瘍は日常遭遇する機会が近年増加している。初期画像診断としてCT，MRIにより，かなりの精度で良悪性の鑑別が可能になってきたが，最近ではFDG-PETや組織生検についても適応の検討がなされている。内分泌活性の点ではコルチゾール産生能の診断とその予後が議論されてきた。診断には1mg DSTがスクリーニングとして有用であり，長期予後については最近の報告で心血管イベントの発生率が少なからず増加してくることが示され，手術適応やフォローアップ方法の再検討も必要と思われる。今後，わが国でのさらなるデータ集積が期待される。
