夜間多尿

要旨　夜間多尿は夜間頻尿症例の半数以上で認められ，膀胱蓄尿障害や睡眠障害といった病態としばしば併存する。本稿では，夜間多尿が単一の原因となっている夜間頻尿の診断と治療について記述する。夜間多尿は夜間尿量が多い状態である。したがって，膀胱蓄尿障害や睡眠障害に強く関連する疾患が併存しない場合，昼間に下部尿路症状を訴えることは少なく，昼間頻尿や尿意切迫感を伴わないことが多い。また，夜間の1回排尿量は昼間と同等か昼間より多い。夜間多尿は排尿日誌をもとに診断される。治療には飲水量の適正化や下肢浮腫を防ぐ行動療法に加えて，合併疾患治療を優先させたうえで利尿薬やデスモプレシンなどの薬物療法が用いられる。
