内科疾患に伴う夜間頻尿

要旨　夜間頻尿の原因は多岐にわたる。これまで考えられてきた泌尿器科疾患だけでなく，多くの内科疾患が原因であることもわかってきた。特に，尿量が多くなるような病態がその中心となる。これらの病態を尿浸透圧や比重で分類した。

　低浸透圧/低比重のものでは濃縮障害が中心となる。薬剤やうっ血性心不全，ADH分泌異常が原因の水利尿や腎髄質ネフロン障害による再吸収障害，また，高カテコラミン血症による腎血流の昼夜バランスの変化による夜間利尿の増加などがある。高浸透圧/高比重のものでは薬剤性や糖尿病にみられるような多尿がある一方で，睡眠時無呼吸症候群（SAS）でも認められる。髄質低酸素状態が尿細管でのNa再吸収を低下させ，Na利尿による夜間多尿になると考えられている。治療においては，これらの病態を把握し，原疾患を考慮した治療法の選択が望ましい。
