重症尿路感染症

要旨　尿路感染症のうち，尿路通過障害の存在や宿主の状態によっては血中に原因菌が移行して尿性敗血症（ウロセプシス：urosepsis）となり，重症尿路感染症として抗菌化学療法や全身管理，症例に応じて外科的治療を行う必要がある。抗菌化学療法においては，抗菌薬の投与前に尿や血液の細菌学的検査を施行し，すみやかに各病態に応じて推定される原因菌に対し広域スペクトラムの抗菌薬を用いたempiric therapyを開始する。細菌学的検査，薬剤感受性試験の結果に応じて狭いスペクトラムの抗菌薬にde-escalationするdefinitive therapyを心がける。
