小児の尿路感染症

要旨　小児の尿路感染には成人とは異なる問題点が多い。乳幼児期早期では，尿採取と検査法の標準化が重要で，上部尿路感染の局在診断には腎静態シンチグラムが有用である。有熱性尿路感染（fUTI）に対して年長児では経口・注射薬のいずれも選択となるが，乳児期早期は注射薬が必須である。基礎疾患として膀胱尿管逆流（VUR）が重要であるが，自然軽快までの保存的治療で予防抗菌薬投与が行われる。VURは唯一のfUTIのリスク因子ではなく，排尿排便障害や包茎の関与も近年強調されている。小児の無熱性尿路感染は，昼間尿失禁の背景となる。小児の精巣上体炎は精巣捻転との鑑別が重要だが，必ずしも尿路感染症とはいえない。
