要旨　前立腺生検のアプローチ法には経直腸的と経会陰的があるが，全世界的には経直腸的生検が大半を占めている。両者は直腸を経由するか否かで大きく異なり，前処置や予防抗菌薬の投与方法に関しては，区別して対応すべきである。

　近年，経直腸的生検後のキノロン耐性菌による感染率増加の問題が深刻化している。経直腸的生検前に腸内細菌群の感受性検査を行うことで検査後の感染率が減少したとする報告が散見され，今後主流となる可能性がある。前立腺への移行が優れているキノロン系抗菌薬をいかにして使用するかが重要である。また，検査後に発熱が生じた際には，すみやかに各種培養検査を行った後に広域スペクトラムな抗菌薬を投与すべきである。
