腎癌における核異型度評価の変遷

▶ポイント

・核異型度は組織型，紡錘（類肉腫）細胞またはラブドイド細胞成分の存在，壊死，微小血管侵襲とともに腎細胞癌の病理学的予後因子である．

・Fuhrman分類は核の大きさ，核小体および著しい多形核，巨細胞，紡錘（類肉腫）またはラブドイド細胞の存在を基準とした4段階分類で，highest areaのgradeを記載する．

・腎癌取扱い規約で用いられている3段階分類は腫瘍細胞核の大きさを，正常近位尿細管上皮細胞核と比較して分類し，優勢度の順に不等号を用いて表記する．

・最近提唱されたWHO/ISUP分類は核小体の大きさと著しい多形核，巨細胞，ラブドイドまたは紡錘（類肉腫）細胞成分の存在を基準とした4段階分類で，highest areaを記載する．淡明細胞型腎細胞癌，乳頭型腎細胞癌に限定して適用し，嫌色素性腎細胞癌などの他組織型には適用しない．
