ロボット支援下腎部分切除術

要旨

 小径腎腫瘍に対する標準術式は腎部分切除術(partial nephrectomy : PN)であり,侵襲性の観点より腹腔鏡下腎部分切除術(laparoscopic PN : LPN)が現在のトレンドとなっている.しかし,LPNは鉗子操作の制限などにより技術的難易度が高く,阻血時間の問題もあり適応症例も限られる.ロボット支援手術は高倍率3D HD画像,多自由度鉗子などの技術革新により鏡視下手術を進化させた術式として急速に発展している.泌尿器科領域ではロボット前立腺全摘除術の豊富な経験やさまざまな改良により,ロボット支援腎部分切除術(robotic-assisted PN : RAPN)において阻血時間の短縮,安全性の向上が認められ,2016年4月には保険適用となった.本邦でも今後ますます増加するものと考えられる.
