ポイント

・腰部斜切開と傍腹直筋切開を合わせて長い皮切を置き，経後腹膜アプローチで行っている．

・リンパ節郭清は，原則全例で行っているが，高齢（80歳以上），合併症のある症例では省略している．郭清範囲は解剖学的テンプレートの基づいて行っている．

・膀胱カフ切除は省略すべきではない．腎盂〜上中部尿管癌では膀胱外アプローチとしているが，下部尿管癌では膀胱を切開し，確実に壁内尿管を直視下に切除している．
