2nd-TURの適応

質問 : T1 high gradeの場合，2nd-TURは必ず施行しないといけないのですか？

▶ポイント

・2nd-TURの適応は，すべてのT1腫瘍で，特に1st-TUR-BTで明らかな不完全切除例や固有筋層が採取されていないものには必須といえる．

・2nd-TURは，1st-TUR-BTからおおよそ6週後までに施行し，1st-TUR-BTでT1腫瘍が検出されたすべての部位を再度切除する．

・T1腫瘍に対する2nd-TURは，T2のステージングエラーを是正する診断的意義，T1 high grade症例のなかでより進行リスクの高い症例を抽出する意義，さらに非再発や非進展生存率を延長する治療的意義を有する．

・しかしながら，T1あるいはhigh grade症例のすべてに2nd-TURを施行するかについては，ガイドラインだけでは片づけられない問題も存在している．
