要旨
尿道の超音波検査は広く普及した検査ではない.尿道が超音波検査の対象になることすら知らない泌尿器科医も少なくないかもしれない.実際,超音波プローブを普通に尿道近傍に接してみても,得られる所見は乏しい.しかし,尿道の内腔に液体がある状態での観察ならば話は違ってきて,いままで見えなかった尿道のシルエットが浮かび上がる.尿道の内腔に液体がある状態で行う超音波検査の手技を「ソノウレスログラフィー」という.ソノウレスログラフィーの原法は排尿時の観察であるが,われわれは逆行性にゼリーを尿道内腔に注入した状態で尿道の形態を観察するソノウレスログラフィーの変法(modified sonourethrography : mSUG)を開発した.mSUGにより,正常尿道の観察のほか,尿道の狭窄や損傷など病的な状態も観察することが可能である.形態の観察だけでなく,尿道カテーテル留置の補助に用いることも可能なので,mSUGの用途は幅広く,得られる所見も有意義である.本稿では,尿道の超音波全般の現状を示しながら,ソノウレスログラフィーの歴史と,われわれが行っているmSUGを用いた取り組みを概説する.
