▶ポイント

・尿路結石による尿路閉塞を伴う急性複雑性腎盂腎炎の原因菌の約3/4はグラム陰性桿菌であるため，初期抗菌薬はグラム陰性菌に強い抗菌力をもつ抗菌薬を使用し，その後薬剤感受性に従いde-escalationする．

・上部尿路に対するドレナージは尿管ステント，腎瘻ともきわめて有効である．

・全国調査による本疾患の死亡率は2.3％であり，合併症を含めた全身管理が重要である．
