▶ポイント

・脳卒中における排尿筋過活動の発症機序は，脳幹部橋排尿中枢に対する前脳からの抑制性投射が障害される脱抑制のみでなく，脳幹に対する促進性投射の亢進も原因とされている．

・特に問題となる過活動膀胱以外に，下部尿路閉塞，認知症における機能性尿失禁や心因性の頻尿なども念頭に置く必要がある．

・早期の原疾患に対する治療，リハビリテーションによるADL拡大の双方が，下部尿路機能障害軽減につながる．
