Point

●LRPでは，トロッカー挿入位置を工夫することにより腹部手術既往例にも適宜対応可能である．

●膀胱頸部離断は側方からのアプローチを併用し，なるべく膀胱頸部を温存している．この際，内尿道括約筋の縦走筋構造にも注目する．

●神経温存手技では，エネルギーデバイスはできるだけ使用せず，クリップを使用して行う．Lateral pedicleから流入する動脈は確実にクリッピング後に切断する必要性がある．

●DVCの切開は，バンチングは行わず鋭的切開時に動脈の断端のみをモノポーラで凝固し，すべて切断後に縫合閉鎖することで周囲への侵襲を最低限に抑えている．

●尿道の離断は側視を使用して，尿道の2層構造を認識した離断を行っている．
