▶ポイント

・副腎皮質癌は予後不良であり，診断および治療に際してきわめて高い専門性が求められる．通常の副腎皮質腺腫の術後に副腎皮質癌と病理診断された場合は，リスク因子や病理結果にてフォローアップの内容が異なる．診断時にすでに副腎皮質癌が強く疑われる場合は，経験のある専門施設へのコンサルトが肝要である．

・近年，副腎皮質癌に関与する遺伝子変異や治療標的薬および免疫療法の報告がなされているが，現時点では予後改善のためにはさらなる臨床上の発展が必要な状況である．ENSATの診療ガイドラインが2018年に改訂され，診療方針の変遷が注目される．

・副腎皮質癌患者の予後改善のために，内分泌内科医，病理医，放射線科医および泌尿器科医（外科医）の連携が重要である．
