限局性前立腺癌に対する手術と外照射放射線療法の比較

要旨

 手術と外照射放射線療法は,限局性前立腺癌に対する同等の治療選択肢とみなされている.既報では長期生存は手術が勝る可能性が指摘されているが,両者の比較においては異なる患者背景や副作用プロファイルなどの諸問題があり,単純な成績比較は困難である.また,各治療でロボット手術やIMRTといった技術革新も進んでいるため,治療成績をアップデートしていくことも重要である.最近,手術と外照射放射線療法を比較した初のランダム化比較試験の結果が報告された.また,筆者らは最近,両者の先端技術同士の比較論文を出版した.これらの紹介も含め,限局性前立腺癌に対する両モダリティの比較について腫瘍学的成績を中心に概説する.
