▶ポイント

・「人生の最終段階における医療の決定プロセスに関するガイドライン」の改定に伴い，アドバンス・ケア・プランニングを含む意思決定支援の重要性が強調されている．

・意思決定支援においては，医療の実施/非実施の希望を文書や記録に残すことだけではなく，医療者と代理決定者が患者の心理社会的・情緒的背景を理解し共有するプロセスが重要である．

・アドバンス・ケア・プランニングは，患者の予後から推測される適切な時期に，患者の病状認識・話し合うための心構えなどを見極めて開始する必要がある．
