▶ポイント

・化学療法関連の悪心・嘔吐（CINV）に対する制吐薬として，5-HT3RA，D2RA，NK1RAが有効であると考えられており，制吐薬のガイドラインはこれらの組み合わせが推奨されてきたが，昨今MARTAであるオランザピンの併用も提案されてきている．オランザピンは強力な制吐作用が期待されるが，高血糖，過鎮静，便秘，静座不能などの副作用管理も重要である．

・好中球減少状態で，発熱から5日未満の初期において問題となる微生物は，緑膿菌をはじめとしたグラム陰性桿菌であるため，発熱性好中球減少症（FN）の際の初期治療は必ず緑膿菌までカバーを広げた治療が重要である．
