以前の常識

・デブリードマン後の創部は連日洗浄し，浸出液が多ければ頻回にガーゼ交換を行っていた．

・便や尿による二次汚染を予防するために，人工肛門や膀胱瘻などを造設していた．

現在の常識

・デブリードマン後の創部は感染所見に応じて適宜デブリードマンと洗浄処置を行いつつ，局所陰圧閉鎖療法（NPWT）管理を併用する．

・NPWTによって創部の二次汚染を予防できれば，人工肛門や膀胱瘻などの造設を回避できる場合がある．
