以前の常識

・腎移植前の免疫学的リスクは主にcomplement dependent cytotoxicity（CDC）で判定されていた．

・ABO血液型不適合やドナー特異的抗HLA抗体陽性などの免疫学的ハイリスク腎移植では脱感作療法の一部として脾摘が施行されていた．

現在の常識

・抗体検出検査としてフローサイトメーターやLuminexを用いた検査が行われるようになり，免疫学的リスク判定がより高感度に，かつ精密にできるようになっている．

・ABO血液型不適合腎移植でリツキシマブが保険適用になり，免疫学的リスクが高い症例でも脾摘をせずに腎移植が行われている．
