後腹膜線維症（IgG4関連を含む）

以前の常識

・後腹膜線維症の約70％は，特発性後腹膜線維症と診断されていた．

・特発性後腹膜線維症の病因として，腹部大動脈の動脈硬化性プラークに対する局所的な炎症反応が推察されていた．

現在の常識

・これまで特発性後腹膜線維症と診断されていた症例の約半数に，IgG4関連疾患が含まれている．

・後腹膜線維症の診断を進める際には，IgG4関連疾患を疑い他臓器病変の検索が必要である．

・IgG4関連疾患の病因は明らかでないが，自然免疫や獲得免疫の異常などが示唆されている．
