間質性膀胱炎

以前の常識

・間質性膀胱炎はハンナ病変があるものをハンナ型間質性膀胱炎（HIC），ハンナ病変はないが膀胱拡張後の点状出血がみられるものを非ハンナ型間質性膀胱炎（NHIC）と分類されていた．

・頻尿・尿意亢進・膀胱痛などの症状があるにもかかわらず，ハンナ病変も点状出血も認めないものは過知覚膀胱（HSB）と呼ばれていた．

・膀胱水圧拡張術が主な治療方法であった．

現在の常識

・従来の間質性膀胱炎様の症状を呈するものの総称を，間質性膀胱炎・膀胱痛症候群（IC/BPS）と分類している．

・IC/BPSのうちハンナ病変のあるものをHIC，それ以外をBPSとし，2つを明確に分ける必要がある．

・HICは組織学的，遺伝学的にも炎症性疾患といえるが，それ以外のBPSには炎症所見は認められず，2つは別の病態であることがわかり始めている．

・ハンナ病変を有する場合は水圧拡張術は効果が低いないしはほとんどなく，ハンナ病変の切除や焼灼が著効する．
