外傷に伴う尿道狭窄症

以前の常識

・尿道外傷の急性期に尿道カテーテルを留置すると，続発する尿道狭窄症が軽症化して待機的治療が容易になると考えられていた．

・尿道狭窄症には内尿道切開術や尿道ブジーなどの経尿道的治療が第一選択で，再狭窄した場合には経尿道的治療を繰り返し行うことが普通であった．

現在の常識

・急性期の尿道カテーテル留置は損傷した尿道にさらなるダメージを与え，続発する尿道狭窄症を複雑化させるリスクが高い．尿道操作はせずに膀胱瘻造設を選択することが望ましい．

・ごく軽症例を除き，尿道狭窄症に対する最も治癒率の高い治療法は開放手術による尿道形成術である．
