▶ポイント

・StageⅡ―Ⅲ膀胱癌の標準治療は根治的膀胱全摘除術，リンパ節郭清，尿路変向術であるが，適切なリンパ節郭清の範囲（標準vs.拡大郭清）についてはまだ明らかになっていない．

・ロボット支援根治的膀胱全摘除術（RARC）も選択肢である．腫瘍学的アウトカムは開腹手術と同等と報告されている．

・シスプラチンベースの術前化学療法は有効性が証明されているが，シスプラチン不適格症例に対する術前化学療法のエビデンスは乏しく，その有効性は証明されていない．術後補助化学療法の有効性も報告されているがランダム化比較試験ではない．

・社会の高齢化に対応して高齢者・フレイル患者に対する治療選択は重要な課題である．膀胱温存療法のエビデンスも充実してきており，積極的に膀胱温存を図る集学的治療（トリモダリティ治療）も選択肢となるが，症例選択が必要となる．
