高齢者の尿路感染症に対する抗菌薬治療

▶ポイント

・前立腺肥大症や過活動膀胱など年齢とともに増す基礎疾患や，抗菌薬服用歴を有する高齢者の耐性菌などが難治性のリスク因子となる．

・生体反応の低下から典型的な臨床症状を呈さず，コミュニケーションエラーなどで診断が遅れるため，敗血症など重症尿路感染症へ移行することがある．

・治療には腎排泄性抗菌薬が選択されることが多いが，腎機能低下の高齢者では容量調節が必要となる．
