▶ポイント

・高齢者に対する根治的膀胱全摘除術では，根治性よりも安全性を優先し，尿道摘除を省略したり，リンパ節郭清の範囲を限定したりすることで，より出血が少なく短時間で終わることを目指すことも多い．

・尿路変向の方法としては，回腸利用代用膀胱造設術が行われることは少なく，回腸導管造設術もしくは尿管皮膚瘻造設術が選択されることが多い．

・近年では，ロボット支援根治的膀胱全摘除術（RARC）が，開腹手術と比較して，より侵襲性の低い手術として広まりつつある．
