前部尿道狭窄に対する代用組織を利用した二期的尿道形成術

▶ポイント

・高度の排尿障害例では，膀胱瘻造設でurethral restを図り，抗菌薬投与で感染尿を解消しておく．

・狭窄病変の部位・範囲・深達度を，逆行性尿道造影・排尿時膀胱尿道造影・尿道鏡・MRIで評価する．

・一期目手術では，遊離グラフトを結節縫合とキルティング縫合で陰茎海綿体白膜に縫着する．

・二期目手術では，グラフト下の剝離は最小限とし，inverted sutureで尿道形成する．
