膀胱尿管逆流―有熱性尿路感染症に対するファーストタッチ

▶ポイント

・有熱性尿路感染症患児の3分の1余りに膀胱尿管逆流（VUR）を合併する．水腎や急性巣状細菌性腎炎（AFBN）など認めた場合は初回からVCUGの適応である．

・VURの治療方針は，VURのグレードや腎実質障害ばかりでなく，1歳前の乳児期か排尿自立後かなどの年齢的要素と，膀胱腸機能障害（BBD）の有無や二次的要因の有無などの3要素から考える．

・VUR診断に留まらず，逆流性腎症（RN）合併の有無，進行性RNの評価が必要である．
