精巣腫瘍手術の注意点―高位精巣摘除と化学療法後の残存腫瘍切除

▶ポイント

・高位精巣摘除術では，術後腫瘍マーカーの正常化や最低値を確認し，治療の継続の要否を判断する．

・後腹膜リンパ節郭清術では，神経温存を検討し，温存を意識した手術を行う．

・後腹膜リンパ節以外の転移巣も残存している場合は，切除の可能性を検討し，可能な限り完全切除を目指す．

・最後の仕上げである残存腫瘍切除が円滑に施行できるように化学療法中から複数科によるチームを形成し術式や切除範囲を検討してゆく．
