幽門側胃切除後の三角吻合法

要旨：当科では1995年以降，幽門側胃切除術後のBillrothⅠ法再建に三角吻合を導入し，2004年末までに約800症例に施行している．吻合手技のポイントとして小彎縫合線がtriple stapleにならないように吻合線と直角に交差させる方法を中心に解説する．また，当科での同一チームにおける三角吻合と手縫い吻合のretrospectiveな比較検討では，D1郭清で三角吻合群の手術時間が短く，D2郭清でも短い傾向を認めた．縫合不全はなく，吻合部狭窄に関してもCommon Terminology Criteria for Adverse Events v3.0（CTCAE v3.0）のGrade 3以上の重篤な合併症はなく，術後成績も良好であった．
