幽門側胃切除後のUncut Roux-en Y吻合法

要旨：近年，幽門側胃切除後の再建法としてRoux-en Y残胃空腸吻合法が普及しつつあるが，この吻合法にはRoux-en Y stasis syndromeという特有の合併症があり，治療に難渋することがある．そこで，Billroth-Ⅱ法の輸入脚の管腔を遮断することにより，空腸を切断することなく空腸の神経筋伝達路を保持した状態で胆汁・膵液の残胃への逆流を防止するUncut Roux-en Y法が提唱された．われわれはこの再建法を2004年2月より導入しているので，その手技と短期成績について述べる．
