はじめに

　肝癌に対する治療として各種の非手術的治療が盛んに行われるようになった現在でも，局所制御能の点で外科的肝切除術をしのぐものはいまだ存在しないといってよい．しかし，外科的肝切除術は腫瘍の占拠部位や肝予備能によってその適応が厳密に制限されるため，最も制約の多い治療法の1つでもある．また，肝機能，術式などの違いでその治療経過が症例ごとに大きく異なることも本切除術の特徴である．これらのさまざまな可変因子が存在するなかで最大限の効果と安全性を確保するためには，術前・術中・術後を通して整理された一定のプロトコールに則って治療を行うこと，すなわち治療の標準化がきわめて重要であると考えられる．

　本稿では，肝癌を肝細胞癌，肝内胆管癌，転移性肝癌の三疾患に分け，それぞれに共通する点と異なる点を明らかにしながら，教室で行っている肝切除のプロトコールを概説する．
