はじめに

　胆管癌に対する有効な治療手段として，外科的な治癒切除以外にない現状では，まず可能な限り外科切除を目指した診断・治療プランを検討することが重要である^1,2）．胆管癌は解剖学的特性から動脈・門脈に近接して存在することから，正確な癌の胆管進展度（水平・垂直浸潤），血管浸潤度，リンパ節転移などを十分に検討する．また，胆管癌症例では閉塞性黄疸を伴うことが多いことから，特に肝切除例においては肝予備能を十分に評価することが重要である．これらを総合的に検討し，切除の可否を決定し，さらに適切な術式選択を行っている．また，術後病理組織の結果から補助療法の必要性の有無などを決定している．

　本稿では，現在の当科における胆管癌に対しての診断・治療・経過観察において施行しているプロトコールを紹介する（図1）．
